Health Through Action Partnership Program Brochure

This brochure describes the Health Through Action program which is a $16.5 million, five-year partnership between the W.K. Kellogg Foundation and the Asian and Pacific Islander American Health Forum (APIAHF). This is the first such cooperative effort to improve health and reduce health care disparities for the Asian Americans, Native Hawaiians, and Pacific Islanders population. APIAHF serves as the program office and coordinating organization for this initiative.
